To register by mail, print a copy of text only registration form and mail to Iowa State University, University Conference Services, 102 Scheman Building, Ames, IA 50011. Make payment with credit card, check, money order, purchase order, or wire transfer.

Fax
To register by fax print a copy of text only registration form and fax to ISU University Conference Services at 515.294.6223. Make payment with credit card, check, money order, purchase order, or wire transfer.

Change in Plans
If you cancel your registration on or before March 21, 2008, your registration fee will be refunded. No refunds will be made after March 21, 2008. Call ISU University Conference Services at 515-294-6222.

NOTE: Write names of all individuals on the check if one check will pay the registration fee for several individuals.
