To register by mail, print a copy of text only registration form and mail to Iowa State University, University Conference Services, 102 Scheman Building, Ames, IA 50011. Make payment with credit card, check, money order or purchase order.

Fax
To register by fax print a copy of text only registration form and fax to University Conference Services at 515.294.6223. Make payment with credit card, check, money order or purchase order.

Change in Plans
If you cannot attend the conference, you may send a substitute. If you cancel your registration before March 17, your registration fee will be refunded less a $15.00 processing fee. No refunds will be made after March 17, 2008. Call University Conference Services at (515) 294-6222 with any questions.
