To register by fax print a copy of text only registration form and fax to University Conference Services at 515.294.6223. Make payment with credit card, check, money order, or purchase order.

Change in Plans
Registrants canceling by July 25 will receive a refund less a $10 processing charge. No refunds will be made after July 25. Substitutions may be made at any time. Walk-in registrations will be accepted at the late registration rate for the symposium if space is available.
