Registration Options
The workshop fee includes a workshop notebook of resource materials supporting each topic area covered, software CD, morning coffee, break refreshments and lunch for the day. Workshop fees are $150 if participants register before or on June 4, 2007. After June 4th, late registration fees for the workshop are $165. Registrations will be accepted till the workshop is full.

By US Mail
To register by mail, print a copy of brochure, fill out the registration form, and mail to Iowa State University, University Conference Services, 102 Scheman Building, Ames, IA 50011. Make payment with credit card, check, money order, wire transfer, or purchase order.

By Fax
To register by fax, print a copy of brochure, fill out the registration form, and fax to 515.294.6223. Make payment with credit card, check, money order, wire transfer, or purchase order.

Change in Plans
Written cancellation requests received before 5:00 PM on June 4, 2007 will receive a refund minus a $15 processing fee. No refunds will be given after 5:00 PM on June 4th. If you cannot attend the workshop and have paid in full, you may send a substitute. Call registration clerk at (515) 294-6222 if you have a change in plans.
